• And referring clients to inpatient and outpatient programs. Collaborate with CRMC hospital and their SAS navigator's Chiayi or Stephanie and refer their patient to Sud inpatient programs, Saint ,Agnus, Kaiser their navigator Sonja Ayerza .

Responsible for outreach, screening, testing and coordinating client services, giving referral to community resources and also the minority AIDS initiative program for primary HIV care, mental health and substance abuse disorder & other community base providers available to enroll clients.

Develop and implement risk reduction plan to ultimately reduce the contraction and transmission of HIV/AIDS and sexually transmitted diseases/infections.

Conduct health education groups, case manage clients thru residential services and then follow thru outpatient services.

Inputting & analyzing data. Make sure inputting is accurate as it relates to invoicing and billing.

Utilize Microsoft server & learn and utilize intranet database and other databases. Trouble shoot any issues on the data base and intranet used by the Company.

Client tracking and case management per database and caseload. Work closely with clients in keeping them on track with schedules.

Submit reports on spread sheets to Parole on attendance and program completion.
