I am enthusiastically inquiring in hopes of consideration for placement within the HIV department at Shanti Project. I am a community centered self-starter. I enjoy working and helping underserved individuals and families and being of service. I believe I possess a great deal of passion for the field of HUMAN SERVICES and a strong dedication to the advancement of impacted communities. I am a dependable worker who works independently and has good accuracy with work. I am able to follow directions well and is extremely reliable, considerate, and organized. I also have good attendance and am very cooperative and reliable.
As a Bilingual Care Navigator at Shanti PACT, it is my responsibility to build trusted relationships with Potrero Hill residents while increasing housing stability through tailored eviction prevention and advocacy services. I spend lots of my time with my clients preparing them for upcoming relocations. I am responsible of providing up-to-date information, helping residents get their files and vital documents in order to help them transition into their new HOPE SF units smoothly. I currently case manage approximately 40-60 On and Off lease families and Single individuals.
I oversee the EHV program (Emergency Housing Voucher) where I meet weekly with HSH and SFHA to make sure all referred applicants are housed. I have credentials to the ONE system where I am able to monitor and track all EHV applicants voucher status. I also provide my residents with on-going emotional support while building trusted working relationships with San Francisco Housing Authority staff, HOPE SF staff, Potrero project development partners (BRIDGE Housing, Eugene Burger Management) and local community health and social service partners such as the Potrero Hill YMCA, C.A.R.E and the Potrero Hill Wellness Center.
