  • Obtain vital signs and do a complete physical assessment. Currently working in the home of a special needs 5-year-old. I am responsible for all of the ADL's which include, tube feedings, diaper changes, suctioning if needed, and assisting with daily activities as the client is non-ambulatory and nonverbal. During shift I also obtain vital signs, and do a complete physical assessment along with giving medication if needed. I also contact the physician and the supervisor with any change in condition.
