Within 24 hours after the patient's admission, a thorough evaluation is completed to determine and record the expected transition plan for the patient.
To create and carry out a successful transition plan, it incorporates essential components of patient assessment, patient choice, and accessible resources.
For patients who have been recognized as having a high risk of readmission, the complex/psychosocial assessment and plan are completed.
For patients with specified needs, such as mental health, lack of support networks, financial obstacles, end-of-life, and/or medication adherence, psychosocial assessment and intervention are provided.
May assign LVN/LPN or assistant staff to carry out the transition strategy. and monitors the transition plan's completion to ensure it is accurate and on schedule.
Provides patient and healthcare team education regarding resources and benefits available to the patient along with the economic impact of care options
Ensures that the healthcare team provides education to the patient/family/caregiver before discharge.
