As a Support Coordinator I worked with an team of professionals to create person centered plans for individuals with disabilities. I completed assessments to gain knowledge and understanding about what each individual needed and wanted in their lives. I helped individuals set up their very first apartments. I trained, mentored, and worked side by side with support staff to learn and carry out each individuals overall service plan. I developed meaningful and trusting relationships with those supported and their families.

Facilitated regular meetings with team members to discuss progress updates, address challenges, and brainstorm solutions for ongoing success.

Kept program-related data accurate, up-to-date and easily accessible for accessibility.
