The Convenient Care Certified Registered Nurse Practitioner (CRNP) is a professional with education and training in the primary and emergency medical care of pediatric to geriatric populations. The CRNP is responsible for providing primary care to clinic patients under the supervision of the supervising physician, indirectly under the supervision of the Director of Provider Practice Operations, and ultimately reports to the Chief Operations Officer, Heritage Valley Health System.

Provision of advanced practice nursing services through palliation and management of the terminal condition, and develop treatment plans for patients at home and possible other settings which may include: general inpatient units, hospitals, assisted living, independent living, senior housing, and skilled nursing facilities. Communicate significant findings, problems, changes in conditions or environment to the clinical manager/attending physician and other members of the interdisciplinary teams. Complete face to face encounters and assess the following" disease related changes, decline in function, adherence and effectiveness of pain and symptom regimine, the patient and family coping and quality of life. Order and review appropriate labs, xrays, EKG's and other diagnostic procedures for palliation and treatment of the terminal disease as dictated by the patients condition and documents the treatment plan accordingly and reviews results with collaborating physicians.
