I did Case management and home health visits. included in the visits was patient assessment, vital signs, ordered medication refills from pharmacy, set up weekly med planners, dressing changes, IV therapy, communicated with Dr and changes in the Pt. condition, drew lab and transported blood to the labs for processing.
