  • Provides in-home services to children and families to assess protective capacity and maintain children in the family home and support successful reunification.
  • Attends the Child Safety meeting and works with CPS worker and families to develop a safety plan for the children.
  • Conducts the Protective Capacity Assessment with parent(s) or caretakers residing in the home.
  • Makes referrals for specialized DHS services; i.e. Skills Trainer, Parent Training, Bi-lingual Counseling, System of Care, Partnership Team.
  • Makes referrals to appropriate community resources.
  • Works closely with the parents and schools to foster the child's achievement of appropriate academic goals.
  • Provides direction and support to families in appropriately meeting the individual needs of their children.
  • Makes referral for family planning information and pregnancy counseling when appropriate.
  • Maintains regular planned face-to-face client contacts and documents those contacts in FACIS.
  • Develops a case plan, expected outcomes and action agreements based on findings of the Protective Capacity Assessment and any findings of expert evaluations.
  • Involves clients (and supportive extended family members and others where appropriate) to develop the case plan or court ordered conditions. Addresses cultural issues in plan.
  • Communicates clearly with clients about expectations, contacts planned, time lines and benefits and consequences of compliance or non-compliance.
  • Utilizes Safety meetings and FDMS to assist in SOC/SNB case planning and common case plans with other DHS divisions and community partners. Maintains compliance with new HIPAA regulations.
  • Meets regularly with clients to develop and maintain professional working relationships with them which encourages, assists, and empowers them to make needed changes. Maintains availability to clients by phone and field and office appointments.
  • Provides case management services through referrals and regular contact with service providers for parents and children to coordinate services and assess progress.
  • Maintains accurate, detailed, chronological case notes of all case contacts and activity in OR-Kids on a regular/timely basis. Reads e-mail daily throughout the day, changes telephone voice message daily and answers voice messages daily throughout the day. Returns telephone calls within 24 hours.
  • Attends court hearings as appropriate to the case, makes case presentations, prepares concise and comprehensive court reports, testifies and makes recommendations to court when requested.
  • Completes regular narrative recordings at six month intervals. Maintains case file including correspondence, treatment reports, and other documentation. Completes other forms and maintains data
  • Generates referrals and payments and maintains documentation.
  • Attend meetings as assigned by Supervisors including but not limited to: Mandatory Reporter Trainings for Community
  • Attend unit meetings
  • MDT meetings
