Awards

   In 2017 I won the first Matthew Miller Patient Safety Award at Providence Saint Joseph's Hospital. I was awarded this honor due to my diligence in my role as a Medication Reconciliation Technician. A patient had come in altered and unconscious. Due to the severity and the quick deterioration of the patient I immediately performed a med rec on this patient. I found that the patients pill bottles where very old and worn away. The patient was receiving a large amount of Mehtotrexate daily by the caregiver. I found that odd as that is not the normal dosing of this medication. In calling and talking to her pharmacy, it was determined that the Methotrexate the patient was receiving at home was not the patients prescription, but the patients significant others. Due to the worn label, the patient was also receiving a very high dose. I relayed this information to the Pharmacist and the Doctor. The patient was found to have Methotrexate poisoning, and was quickly put on the correct medication to reverse this. The patient made a full recovery
