• Met with the Director of Pharmacy and discussed the topic of professionalism
• Created a SOAP note based on actual patient case while shadowing clinical oncology/hematology pharmacist and presented the case to peers and preceptors at end of the rotation
• Made intervention regarding AKI risk with combination therapy of vancomycin and piperacillin/tazobactam, recommended a switch to vancomycin + cefepime for safer anti-MRSA/anti-pseudomonal coverage

- Worked closely with nurse practitioner and nurse practitioner students to facilitate better health care and patient safety

- Made interventions and recommendations after conducting medication reconciliations for geriatric patients

- Met with patients to discuss care and how to better cater to their needs, participated in patient advocacy by answering questions and educating patients on their treatment
