Nursing in a secured 16 bed men/women facility for those experiencing severe and chronic mental health needs. Collaboration with the treatment team to quickly assess patients and deliver appropriate treatment while managing rapidly changing conditions. Managed care from admission to discharge. Administered medications and treatment to patients and monitored responses while working with healthcare teams to adjust care.

Providing education patients, families and caregivers on diagnosis and prognosis, treatment options, disease process and management and lifestyle options. Training of new nurses in proper techniques, care standards, operational procedures and safety protocols. Provided direct patient care, stabilized patients and determined next course of action. Facilitated groups such as medication education, DBT skills, Illness management and recovery, substance use education and health and wellness education.

Provide home health mental health and medical care. Creating the care plans, disease process education. drawing labs, wound care, administer injections, INR testing, medication education, teaching coping skills and problem solving skills, communicate with physicians and families, physical and mental health assessment, monitor for suicide ideation. Manage on call.

Assisting the provider with clinic, helping clients gain access to health care and coverage, utilizing patient assistance programs, providing education, teaching health education groups, TB testing of staff, managing refill requests, case management, suboxone clinic and providing street outreach.

I worked on an ACT (Assertive community treatment) Team. I provided care for clients managing chronic mental health/substance use needs. Our work is based in the community in clients homes. I managed health assessments, care plans, set up medical care for clients often going side by side, provide med management, give injections, meet 1:1 with clients and focus on treatment plan goals, help teach coping skills, provide health and wellness education, assist with housing case management and ADL's, manage lab and med orders, prior authorizations and on call/crisis line work.
