Applying for Arizona Long Term Care Services and AHCCCS for patients that qualify financial and medically.

Conducting financial and medical interviews and informing IDT team and patients and patient's family member's.

Ensuring that proper documentation has been submitted by the deadline.

Discharge Coordinator

Planning and developing discharge plans that is specific needs for patients to ensure a safe transition from skilled nursing to home or another care facility.

Avoiding any delayed transfers for patients and their care.

Communicating with patients and their family members as well as IDT team on any barriers and inpatient services to have an effective discharge from skilled nursing facility.

Supporting patients with care packages , compile discharge packet, accommodation with referrals for patients with home health, hospice, DME, transportation, and placement, securing signed scripts for medications including narcotics, and coordinate teaching for patients.

Case Manager

Obtaining authorizations for skilled and acute patients, coordinate with patients , IDT team and DC planner, advocate for patients on appeals, update all insurances with patient care and progress from medical to therapy.

Lead weekly skilled nursing review to ensure patients are obtaining skilled need goals to ensure safe discharge.
