By streamlining discharge processes at Waterbury Hospital and VNA, I reduced readmission rates. Educating clients and caregivers empowered them to better manage their healthcare. My commitment to patient communication and care planning ensures high-quality, coordinated care.
CareCentrix was my first telephonic care management position. It proved to be a rewarding position to educate and assist clients with healthcare management and education. By collecting a vast supply of educational and community resources, clients and caregivers had the tools to better understand an manage their health.
This was one of the highlights of my nursing career. In 2010 I was awarded the Florence Nightingale award for going above and beyond for a client in need. She needed to be seen in interventional radiology as her abdominal drain was occluded. If she called 911, she would have been transported to a nearby hospital that was not familiar with her complicated health history.
I re-arranged my homecare schedule for the day and transported her 27 miles to her hospital of choice. After 2hours for successful intervention, we returned home. She was physically and emotionally comfortable and without infection. She remained a lifelong friend until her death.
