  • As a Project Field Manager, Mr. Holt is qualified as a Site Superintendent (SS), Quality Control Manager (QCM), and Site Safety and Health Officer (SSHO). The specific roles are noted for each project below.

  • At all locations, work performed on all tanks includes: tank cleaning, validation of deficiencies inspections and reports, install VITO stilling wells, install ATG and sample/gauging stilling wells, replace chime-to-ringwall sealant, perform out-of-service API 653 inspections and report, strap tanks and generate calibration charts.
  • At Altus AFB Tank 555, provide 100% exterior coatings to include lead abatement and markings (9,700 square feet [SF]), perform MT or PT on intermediate manway toe welds, replace three thermal relief loops on the receipt and issues lines, remove the mechanical tape gage (MTG), replace four roof inspection hatches, install an 8-inch double block and bleed (DBB) valve, replace ENRAF stilling well, repair inspection hatch latch assembly, inspect/report on internal floating pan (IFP), replace davit arms on two manways, install additional 36-inch manway on the shell first course, replace high level alarm (HLA)/high-high level alarm (HHLA) and provide new shell mounted HLA/HHLA/high level valve (HLV), replace low level alarm (LLA)/low-low level alarm (LLLA), replace product saver tank (PST) piping systems, replace tank grounding connections, install stairway grounding bar, repair issue line anti-vortex plate and pipe supports, install an external 4-inch DBB valve at the tank shell on the low suction external line, repair internal water draw off line, replace all interior pipe supports, and remove AFFF system.
  • At Malmstrom AFB Tanks H-1 and H-2, provide 100% exterior coatings to include lead abatement and markings (7,865/7595 SF), relocate/replace four overflow vents, replace three thermal relief loops, remove the MTG, repair existing lower manway by replacing the davit arm and cross bar and install a filler drum, repair two grounding cables, install two additional grounding cables, replace PST system to include tank shell nozzles, valves, product return pump, and piping, replace internal water draw-off piping and supports, replace receipt diffuser and supports, replace internal issue pipe supports, install an anti-vortex plate, replace the 3-inch low suction line with new 4-inch low suction line to include supports, replace HLA/HHLA and provide new shell mounted HLA/HHLA/HLV, replace LLA/LLLA system, install HLA platform, repair spiral stairway treads, install four perimeter roof inspection hatches/vents, remove the existing manual gauge hatch and stilling well from the manway cover, remove the center roof vent with manual gauging hatch and install new 24-inch center roof vent, remove center stilling well and repair IFP penetration, install new IFP panel, inspect the tank’s fixed roof rafters for bowing, and remove AFFF system.
  • At Tank H-1, remove six aluminum striker plates and grouting, install six IFP leg caps, provide two scaffold supports on tank roof, and repair internal coatings failures.
  • At Tank H-2 provide 100% new interior coatings (7,100 SF), replace the 12-inch issue piping ball joint gasket and seals, perform Piping Flexibility Study on existing pipe supports in containment area, remove hanging pipe supports and davit arm and shell supports for issue and receipt lines, perform MT or PT on repad welds remaining on tank shell and provide repairs to pinholes, install two pipe supports for the issue and receipt lines, remove current support brackets and perform NDE, install new seal welded repads between tank bottom plates and new brackets and perform MT or PT and VB of welds, perform MT or PT, VB of welds on tank bottom lap and corner welds, repair corner-welded pinhole previously identified, perform MT or PT, and VB on welds at tank sump after center stilling well is removed, install ball valves and caps for the telltales in the tank ringwall, fully seal weld four platform supports attached to the tank shell, inspect and seal weld anchor chairs and shell vertical seams, and replace manual gauge stilling well with an aluminum ladder.
  • At Minneapolis-St. Paul JARS Tanks 3 and 4, provide 100% exterior coatings to include lead abatement and markings (6,800/5,220 SF), remove MTG, replace failed sealant on top of the dike wall joints (700 LF each), replace HLA with HLA/HHLA/HLV and LLA and LLLA systems, install one each scaffold support on tank roof, install stairway grounding bars, replace dike wall penetration boot seals and clamps (4 each), remove the two each 8-inch pressure/vacuum vents (PVs) on the tank roofs and install patch plates, design and install dike wall and piping crossover stairs (3 total).
  • At Tank 3, remove the 4-inch gauge hatch on tank roof and repair with patch plate, remove gooseneck on the 6-inch fixed roof center vent and replace with 24-inch center roof vent, replace spiral stairway, remove two 4-inch blind flange nozzles from the tank shell and repair with patch plates, replace and grout six pipe supports located within the secondary containment, replace roof manway, install four new roof inspection hatches, remove the 6-inch gooseneck vent and install a 24-inch center roof vent, repair three grounding cable connections and install one additional grounding cable, replace internal pipe supports, install an anti-vortex plate, install 4-inch low suction line with internal pipe supports and exterior DBB, and provide 100% interior coating repair for the tank shell, bottom, fixed roof, and rafters.
  • At Tank 4, install HLA platform and provide new roof guardrails on upper roof platform, replace IFP, repair secondary containment’s concrete cracking, spalling and caulking failure, replace seven containment pipe supports, install top platform toe board, install manway davit arms, replace four grounding connections, repair cathodic protection lead wire, modify the internal issue and low suction pipe supports, replace water draw-off pipe supports, provide 100% new interior coating repair for the underside tank fixed roof and rafters (1,000 SF), install new aluminum internal ladder, replace 2-inch DBB valve on the water draw-off exterior piping, install bonding straps on two roof access hatches, install four roof inspection hatches, provide updated tank data plate, and remove one unused blind flange nozzle from the tank shell repair with patch plate.

  • Work performed on both tanks includes: clean tanks, provide grease caps on tank valves, repair internal and external issue and receipt pipe supports, provide a round anti-vortex plates, rework the body cavity relief tubing to drain to the pipe side of a tank valve, replace the high-level control valves, repair low suction pipe supports, replace thermal relief system on the issue and receipt piping, replace six each internal pipe supports, remove and scrap mechanical tape gauges, replace low level and high level alarm systems, rework existing staircases and platforms at the level alarm system to meet OSHA clearance requirement, provide spiral stair grounding, install grease fittings on davit arms, repair water draw off system piping systems to include replacing piping, supports, shell nozzles, and valves, provide and install a stainless steel covers for the water draw-off system motors, provide and install new vent for the product saver tanks, relocate the existing top rail chain to the mid rails at the locations of the roof perimeter inspection hatches, replace chime sealant, repair containment cracks and spalls, provide exothermic welded connections at nine (total) external grounding lugs, provide and install three each scaffold cable supports, provide/install data plates, repair internal and external coatings to include installation of tank markings, strap tanks and generate calibration charts, and perform final API 653 inspections and generate reports. Additionally, for Tank 2, repair the internal floating pan penetration seals and grounds.

  • Remove two dilapidated storage buildings (12x24-foot and 14x22-foot), assembled two new replacement storage buildings. Demolished a car parking lot canopy and associated solar panels. Installed new 135x22-foot canopy, and installed a solar panel power system on the new canopy and on the adjacent grass area to include two 125-foot long and four 65-foot-long solar panel arrays.

  • Performed pipeline modifications on a fuel pier per API 570 to relocate two refueling points that were outside of containment and over water. The carbon steel piping and components were reconfigured to be inside the existing containment. Work included installation of welded and bolted joints, appropriate nondestructive examinations, and coating repairs. Installed 75 feet of 6-inch crossover piping with isolation valves and pressure relief loops and high point vents. Work included welded and bolted joints, appropriate nondestructive examinations, and coating repairs. Cleaned, inspected, and recoated the interior of a Fuel System Icing Inhibitor tank, and installed a new pump and piping to the injection point. Added pressure relief loops to pipeline pig launchers. Cleaned and inspected two lube oil tanks in preparation for a product change.

Repaired tank stairs per API 653 by cutting the stairway off the tank shell and reinstalling the stairway on newly installed brackets on repads. Performed coatings repairs at repads/brackets.

  • Replaced the bottom of Tank 4 by abandoning the existing bottom in place, installing a liner, sand and cathodic protection and slotting in a new bottom with center sump. Work included a complete rebuild of the internal and external tank piping, nozzles, valves, and typical components, as well as complete internal/external coatings. Repairs were also made to Tank 1 to bring both tanks in compliance with API 653, to include new tank float system, new grounds, new roof nozzles, and new Varec systems. A new facility support generator was installed which necessitated removal and replacement of a generator facility wall.
