- Perform comprehensive patient assessments to identify clinical status, recognize emergent conditions, and prioritize care based on acuity
- Collaborate with physicians, case management, and multidisciplinary teams to coordinate admissions, transfers, and level-of-care placement
- Support patient flow and throughput by facilitating timely communication and transitions across departments and external facilities
- Utilize clinical judgment to assess medical necessity and appropriate placement, with working knowledge of InterQual criteria
- Communicate effectively with internal and external stakeholders to ensure accurate exchange of patient information and continuity of care
- Manage critically ill patients on ventilators, vasoactive drips, and CRRT requiring advanced monitoring and rapid intervention
- Function in a charge nurse capacity, assisting with assignments, workflow prioritization, and escalation of high-acuity situations
- Maintain compliance with HIPAA, EMTALA, and regulatory standards while handling confidential patient information
