The Ascent Team Leader is an integral member of the First Episode of Psychosis (FEP) Program, providing comprehensive mental health and dual diagnosis services for individuals who have serious and persistent mental illness.
The Ascent Team Leader works with clients to develop client-driven goals and creative ways to help clients meet these goals.
Services are provided in settings where clients are most comfortable; at least eight (80) percent of services are provided off-site, mostly in clients' own homes.
The Ascent Team Leader provides cross training to other team members and helps ensure that all team members are aware of and focused on helping clients work on their recovery goals, including wellness and transition goals.
The Ascent Team Leader receives training and support from other specialists in the areas of vocational rehabilitation, family therapy, mental health assessment and general health and wellness issues.
All team members are expected to function in a trans-disciplinary role.
