· Worked under direct supervision of the assigned Nurse Manager.
· Collaborated with nurses and other interdisciplinary team members within Veteran healthcare.
· Assigned to inpatient units and huddled with the charge nurses to determine patients with indwelling devices, restraints, a high risk for pressure injuries, and other high-risk patients. Prioritized rounding on these patients first while completing the activities.
· Documented frequency of interventions, patient education, phone communication and all other pertinent to data in CPRS, VISTA, ACUSTAF.
· Applied topical medications as outlined in the use of unlicensed assistive personnel.
· Transported patients to diagnostic procedures, outpatient areas and inpatient wards. Transported patients on telemetry with an Advanced Cardiac Life Support (ACLS) certified RN.
· Promoted early ambulation as appropriate in all inpatient areas as well as the Emergency Department.
· Communicated with the Registered Nurse and or provider prior to removing patient monitoring devices for procedures or activities document.
· Served as Wound Care Champions on assigned unit, assisting with treatments at least every two hours to assist with turning and repositioning of patients. Ensured pressure ulcer prevention measures are in place as indicated for Wound Care.
· Assisted in behavioral/psychiatric situations including crisis interventions, one- on- one observations, and assist with maintaining patient safety in Mental Health.
· Frequently worked with IV therapy to help draw labs, start PIVs, obtain blood cultures, stock carts, and work in the outpatient lab.
· Obtained patient vital signs, calculated EWS score per hospital policy and elevated any concerns to Registered Nurse and initiated CAT (Crisis Action Team) call if needed. Charted Vital Signs using CPRS (Computerized Patient Record System) and the dynamaps device. Able to perform vital signs including, blood pressure, temperature, oxygen saturation, respirations, assess pain level and take weight measurements.
· Ensured timely and accurate documentation into the CPRS by utilizing Nursing Progress Note, Patient Care Activity Flowsheet, Patient Care Critical Note and Patient Admission Part 1. Utilizes bedside documentation including I&O’s, EWS/vitals, hourly rounding sheets and restraint flow sheets.
· Performed EKG, bladder scan, accu-check (blood sugar readings) and placed telemetry monitoring as ordered. Elevated results to appropriate staff with need to know, completed appropriate documentation.
· Initiated suicide and safety precautions. Performed room checks to ensure patient safety, frequent documentation, placed patient in appropriate clothing, observed patients on 1:1 status, completed additional safety checks when a concern arose.
· Promoted patient cleanliness by ensuring patients received bath/shower based on their needs. Aided patients with toileting based on method used (brief, folly, condom catheter, bowel movement system, toilet, bedside commode, bedside urinal). Performed oral care as needed.
