Performs independent, comprehensive telephone and family crisis, and facilitates care coordination through a multidisciplinary approach. Establishes priorities for patient care and evaluates patients' progress toward high level for comfort. Provides patient and family education as an integral part of the services offered. Acts as resource social worker, triages and evaluates patient and family members to assess the severity of the patient's terminal diagnosis and the need for prompt intervention. Communicates with providers regarding determinations and situations, and documents events appropriately. Performs initial psychosocial assessment to evaluate the emotional history and determine health care needs. Makes psychosocial assessment of patients to identify significant cultural practices, the ability to understand and cooperate, and expectations of care coping mechanisms.

➢ Responsible for coordinating a team of staff to assure the continuity of high quality care to home health patients assigned to the team's area.
➢ Initiates and coordinates the patient plan of treatment through the contacting of community services, therapists, and/or social workers, and through staff assignment to provide patient care as ordered by the physician.
➢ Assists the Director of Nurses and Branch Manager in planning, implementation and evaluation of in-service and continuing education programs for staff
➢ Participates in Case Conferences, Mock Survey for JACHO accreditation and shadows mentor Quality Assurance, Utilization Review, and the overall Performance Improvement plan, program, and process.
