•Documenting member medical records and maintaining electronic case management assessments for immediate and future use
•Identifying development areas, including individual member enhancements and overarching LIFE Center opportunities for growth and expansion, then developing and implementing these improvements
•Counseling families in both individual and group settings, and leading relevant activities to educate these care participants
•Visiting in-home members, assessing needs and developing/implementing comprehensive speech/language care plans accordingly
•General administrative duties, including completing and submitting 1768 documents to required agencies within 48 hours
Implement, monitor, evaluate, and update ISP’s, WRAP plans, Behavioral Support Plans, teaching plans and data sheets. Conduct in-home assessments and administer psychological tools to assess each individual’s specific needs. Assist in developing support systems, based on the needs of individuals, by identifying and coordinating a variety of available and applicable services. Supervise a team of Community Support Professionals. Create and incorporate crisis intervention plans and complete HCSIS reports. Assure that administrative records and reports are established and maintained in compliance with policies and regulations. Comply with all policy and procedures, guidelines, standards, laws and contractual obligations.
Supported adults on the autism spectrum habilitate in the home and community. I coordinated with our supports coordinator to help our participants acquire jobs and volunteering opportunities. I collected data daily to help with the constant assessment of our participants regarding their treatment plans and goals. I worked with our Behavioral Health Specialist to complete Functional Behavior Assessments when necessary. I helped the participants monitor their medical health and appointments. I assisted participants and families if and when they are facing legal problems.
