Provide service coordination and support to residents of HRA housing programs and community centers, as assigned. The primary role of the Tenant Service Coordinator is to assist people in connecting with resources for housing stability. This includes maintaining accurate records, direct service resource and referral, and managing community center programming. I also have 2 to 3 interns each year from the UMD Social Work Program that I work with and follow their individual education plan and supervise them as they learn through the school year.

• Collaborated with community and regional partners to provide mobile crisis services and residential crisis stabilization for adults in mental health emergencies, enhancing psychiatric stability and personal adjustment while equipping individuals with skills for independent living.

• Collaborated with tenants in HRA public housing to connect them with services, supporting their journey toward self-sufficiency and housing stability. Coordinated with community agencies and service providers to establish direct service linkages for residents. Consulted with residents, property managers, and HUD to align service delivery with resident needs and enhance understanding of available resources. Applied Trauma Informed Care principles in interactions with tenants.

• Facilitated individual support, advocacy, and housing access services for people with disabilities, enhancing their ability to secure independent living. The primary role of the Housing Access Services Coordinator is to assist people in moving to a home or apartment of their own, in typical community/neighborhood settings. Employees are expected to demonstrate competence in the area of multi-cultural communication (written & verbal), problem solving, flexibility, job knowledge in social services, time management, interpersonal relations, ethics, and communication skills. Major responsibilities: will seek and locate suitable, affordable, accessible housing. Coordinated person-centered planning sessions for individuals regarding housing and moving, empowering them to make informed decisions. Guided individuals in reviewing and completing rental applications and lease agreements, ensuring compliance and increasing successful housing placements. Meets and negotiates with landlords and property staff. Assist individuals with developing household budgets. Assists individuals with locating resources for furnishing their home.

• Wrote treatment plans, met with clients in the community or home and helped them establish goals as well as work on their goals. Role-model appropriate behavior and encourage positive expectations and support. Assist in recovery from mental health issues and educate on mental health issues they are experiencing.

• Handled cash transactions and managed money effectively. Assisted customers with finding the perfect shoe and perfect fit by measuring feet along with searching on line if needed for the best match.

• All the responsibilities of a direct support professional as well as screen new referrals for placement and assure all paperwork is completed and on file meeting licensing requirements. Notify licensure within 5 days of any admissions/discharges and staff changes. Develop with the client and extended support team a person-centered individual support plan identifying personal goals, needs, vulnerabilities and identifying expectations of staff in supporting them. Hire and train staff meeting licensing requirements, assuring appropriate paperwork is on file. Monitor medication administration as defined by our policy and licensing requirements, fill pill boxes on a weekly basis and assure MARS are documented on accurately. Provide daily documentation of services provided and monitor direct staff’s documentation for appropriateness and implementation of support plan as written. Maintain confidentiality always, make schedule and budget for the whole house as well as each client and much more.

• Assure safety, health, proper supervision/monitoring of residents according to individual and facility risk management plans and provide supports for the basic physical care needs of the residents as well as supervision in the home and community, provide accurate and comprehensive documentation of all the above items per house policy and procedures.
