North Carolina CHA, Veronica Brewer, was asked to visit a SNF to meet with an engaged member and help set up an PDV by fully educating the patient on the Landmark program. This patient had been hard to reach and was a high hospital utilizer. Veronica was able to not only meet with the patient but also obtain his discharge details and schedule his PDV with the Landmark provider. She then met with the facility social worker and conducted an introduction meeting. The social worker was pleased to learn of the Landmark program and since that initial meeting, the social worker has been calling the CHA to inquire about eligibility for other members. Landmark now has a strong working relationship with facility. The social worker provides necessary details for home follow up and together we have been able to secure appropriate discharge services for our mutual patients. This not only decreases their likelihood of returning to the hospital, but it also increases their overall quality of life.
