I am a case manager with long term care. I have a mixed case load of assisted living homes, elderly and pediatrics. I work with members and families to ensure they are receiving the services they need.

• I am currently a Case manager of SMI clients at Windsor outpatient clinic
• I complete treatment plans, assessments, intakes, coordination of care with outside agencies.

• I was previously working in a level II substance abuse/seriously mentally ill 60-day treatment program
• I observe client’s in the self-administration of their medications, contact pharmacies, pick up medications, and as well as the safety liaison
• Prior to this I was a case manager at COPE of SMI clients for almost 4 years.
