Summary

As Quality Assurance Director at Synergy home care I found the most effective way to achieve both client satisfaction and ensure caregiver support was in establishing relationships and being available to listen and address issues as they came up. I find teamwork to be rewarding and believe effective teamwork is necessary for providing excellent care. Service audits provided all parties an opportunity to let us know what aspects of our service were meeting the needs of both caregivers and clients and where we could improve.

At US Vets, in order to assist the veterans, developing and establishing trust was key. The personal skills required were listening and meeting the veterans where they felt comfortable; working from there to establish trust in both the program and myself.

My role on their team was to support them in their medical needs.

Both roles were invaluable in increasing my knowledge, experience and understanding of how to meet the needs, in a compassionate way, of individuals who come under our care.

  • Contacted new and ongoing clients to review our service for client satisfaction and to inquire about possible changes in condition, additional services needed an overall satisfaction with both caregiver and services being provided.
  • Conducted audits required by some accounts for compliance for continued benefits.
  • Investigate complaints reported by either caregivers or clients or family members regarding some aspect of service. Seek to resolve concerns reported by caregivers, clients or family members.
  • Service improvement. File a report upon completion of investigation with recommendations for service improvement.
  • Support caregivers with ongoing/challenging accounts by working with the additional healthcare teams, home health and or hospice, to ensure our caregivers are kept informed as healthcare needs change. Keeping the family informed as needed.
  • Report to office staff on a regular basis of accounts that need close supervision and monitoring

  • Assigned 10 special-needs veterans. Assisted the special-needs veterans with making their medical appointments, and arranging their transportation. Keeping track of their medications and placing orders for refill and/or contacting physicians for new prescriptions.
  • Daily check in with the special-needs veterans. Reminders given of upcoming appointments and daily assessment of veterans.
  • Monitored and recordedvital signs and reported concerningchanges to the veteran’s VA medical team.
  • Attended medical appointments with the veterans with various providers. As the veteran’s representative, notes were taken; these notes were charted into the veteran’s file.
  • Regular reporting to the US Vets care team and, when appropriate, to the VA medical team of any changes in condition, refusal of medications and any other matters of concern regarding the special-needs veterans.
