● Is instrumental in the development and annual review of the organizational strategic plan. Supervises Client Care Coordinators, Schedulers, Staffing Coordinators and Caregivers in the office and field.
● Conducts intakes and in-home assessments. Onboards new clients and visits existing clients. Supports clients’ in-home care needs as needed with emergency fill-ins and visits.
● Manages client care coordination and the development of the plan of care. Is responsible for reviewing all admissions and implementation of the plans of care, ensuring that all are within the scope of home care regulations.
● Monitors client progress, coordination of services and expected outcomes. Assists and supervises staff in communication with the client's medical care team. Is responsible for ensuring employee/client care assignments provide adequate and appropriate client care during all service hours.
● Participates in person in interviewing, orientation and selecting new employees. Directs and evaluates personnel in the performance of their dues. ● Represents the agency in person at community functions and participates in public speaking engagements to various organizations, physicians’ offices and other interested pares in a positive and professional manner. Amends trade shows and events on behalf of the Company.
● Manages ongoing record review and initiating changes needed to maintain records in accordance with California and Federal regulations, ensuring all records are up to date, organized and ready for inspection at any me.
● Is instrumental in the development of educational programs through in-services and continuing education programs. Participates in in-service programs.
● Communicates with long term care insurance companies regarding client’s visit's schedule and reimbursement for the same.
● Performs other dues as requested.
Maria "Angel" Souxouay, licensee, owner of Serenity Home Care is a licensed Administrator for Residential Care facility for Elderly and Adult Residential Facility. Angel is originally from the Philippines. My family has been in the health care field for over 20 years. I started as live in caregiver caring for Adults with developmentally disabilities. I was also a CNA worked in different settings, private homes and skilled nursing facilities. I also attended Carrington College in San Jose, Ca where I completed Vocational Nursing.
