* Provided physical and psychosocial assessments for new mothers and babies, breastfeeding assistance, medication teaching and referrals to community resources in the home care setting.

* Informed physicians (OBs & Pediatric offices) of any changes to patients' health (e.g. elevated BPs, weight loss >10% for infants, wound dehiscence, etc).

*Identified patients at risk for domestic violence; emotional problems (depression, postpartum depression, anxiety, etc.); alcohol, cigarette and substance abuse; and patients with financial difficulties.

* A strong advocate for the importance of a healthy diet, regular exercise and controlling excessive weight gain during and after pregnancy.

* Conducted yearly health screenings (weight, height, BMI checks, dental, scoliosis, etc), vion and hearing screenings.

* Attended IEP meetings, developed Individualized Health Care Plans, obtained doctors orders, taught and trained unlicensed personnel regarding proper medication administration and specialized health care procedures.

* Performed classroom visits and assessed students' health issues, provided first aid and notified parents, prn.

* Conducted home visits to assess new, incoming students' health needs.

* Collaborated well with teachers, program directors, SLP, OT, PT and parents in resolving any student issues and concerns.

* Responsible for the preparation of the OR suite, checked on supplies, instruments and machines.

* At times, I was floated to the Maternity Ward to assist with Mother/Baby care, newborn screenings, etc.

* Provided care to acute mentally-ill adults; performed completed physical, mental and emotional assessments, administered medications, monitored for adverse effects, and educated patients of their condition, treatments and patient rights.

* Collaborated well with physicians, case managers, social workers regarding patients' care, condition and other issues.

* Provided assistive nursing care in the Ortho-Diabetic, Spinal Cord Injury and Liver Disease Units; monitored and documented patients' VS, I&O, diet intake and activities.
