
Professional caregiver, support staff, job developer, medical support staff, and educator. Supporting individuals with disabilities and medical needs for over 10 years under different job titles. Supports would initiate anything from hands-on, attending home visits, phone, and virtual visits. With a wide range of support staff, a care plan/service plan was created to best meet individuals' needs, along with providing resources and next steps for their success. Assisting individuals with daily activities, hands-on job support, personal needs, basic medical care, and education. From scheduling appointments, coordinating meetings with medical personnel, SLPs, psychologists, doctors, parents, and teachers. Coordination of activities, lessons, care plans, and health support would be scheduled by the provider, along with reminders of these appointments. Answering questions and addressing concerns during these meetings. For complex situations, additional support may be brought in. Routine notes of observation and conversations would be documented on needs, concerns, and progress made for an accurate record, and data of each interaction would be kept. Data and medical records would be used for further screenings, assessments, and referrals for complex situations or further supports that may be needed.
