• Conduct proactive review of the medical record to ensure timely, accurate and complete documentation and coding.
• Track and trend information used for measuring and reporting provider outcomes.
• Identify missed documentation, compliant query opportunities and areas for PI initiatives.
• Work in collaboration with the coding department to provide education to providers with a focus on value based care, risk adjustment, documentation requirements and HEDIS measures.
• Monitor changes in law, regulations, rules and code assignments that impact documentation and risk adjustment data validation.
• Partner with HCC coders and leadership to complete and track all patient assessment forms as requested by health plans.

Provide transitional care management to patients following a hospital admission to ensure quality patient outcomes.

Provide nursing care to patient presenting to the emergency department with experience in triage, trauma and critical care.

Provide nursing care to patients requiring acute behavioral services on the adult psychiatric unit, child/adolescent psychiatric unit, chemical dependency/ acute detox unit and in behavioral emergency services.
