Program Description: The program is designed to foster the prevention and treatment of chronic back pain at a custom level. Looking at the entire well-being and health of individuals. Including nutrition, mental health, and well-being. Fostering at-home therapy customized to promote a healthy lifestyle and examine movements and physician treatment plans. Members work under the direction of Registered Nurse case managers who facilitate treatments with PT and PT assistants all implemented virtually and telephonically.

Position Summary: Quality clinical supervisor/case manager facilitated the development and management of the design, implementation, and evaluation of The Back Pain Guide program. Created templates used to launch the platforms to support program needs for the clinical staff. Guided performance expectations for clinical staff audits and call reviews. Provided training for new hire clinical staff. Made outbound calls/inbound calls to members to evaluate eligibility/health status for participation in the program. Assessed high-risk members. Prepared and assessed members' SpineZone and PT evaluations. Provided ongoing support for members during program participation. Fostered program growth from the first member to well over a thousand members with additional programs platforming onto the program.

Program Description: Disease management program: Program designed to reduce high-risk members and reduce the need for hospitalization through lifestyle modification, physician communication, and accurate understanding of treatment plans.

Position Summary: Transitioned into an array of different roles throughout employment: Registered Nurse Case Manager, supervising team leader, program developer, and lifestyle wellness coach specializing in nutrition and weight loss/mental well-being and pediatric migraine management team lead. Assessed current patient status, outbound and inbound call team. Correlated pediatric migraine program onto the current platform, adjusted treatment plans, provided education, collaborated with team/medical directors, evaluated service's efficiency, and documentation, and managed telephonic devices.

Program Description:

Passport Case Manager

• Educated patients, families, and caregivers on diagnosis and prognosis, treatment options, disease process, and management and lifestyle options.
• Managed care from admission to discharge.
• Maintained and managed patient caseloads, facilitated passport billing, and supervised homecare aids and services. Management of telephonic devices for prevention of hospitalizations for COPD, heart failure, and diabetes clients.

Wound Care Specialist

• Worked with certified wound care specialists to manage non-healing lesions using different treatment plans such as wound-vac with NPWT, closure device, and bio-surgical care.
• Provide treatment plans, which may include medication management, group therapy sessions, in-home counseling sessions, or other forms of treatment.

Behavioral Health Case Manager

• Perform intake assessments to determine patient needs and risks for suicide, violence, or self-injury. Create and update treatment plans based on patient progress and compliance with treatment goals. Provide counseling, treatment, rehabilitation, case management, and support services.
• Provided education geared to help manage specific patient needs to maintain independent care in home and reduce hospitalization outcomes.

Newborn infant high-risk home health

• Assessed for community resources. Managed “Healthy Start” program to monitor healthy development in newborn/premature infants after hospitalization. Provided nutritional education and weight checks for at-risk newborns

Position Summary: Charge Nurse for labor/delivery, postpartum and nursery

• Assisted in vaginal and cesarean births, treatments, exams, monitoring, and pre/intra/post-operative care, consistently providing healthy outcomes for mothers and babies with high-quality compassionate care. Supervised and rotated all areas of maternity floor including nursery and postpartum care.
• Floated throughout facility to assist in short-staffed units such as: emergency room, intensive care, oncology, medical-surgical, and pediatrics.
