My responsibilities include, but are not limited to,

· Assuring that my clients become knowledgeable of what the Early intervention program is.

· Assuring that they understand the benefits of the program, providing them with the laws and guidelines of the program set fourth by the state and the department of mental health and hygiene.

· Assisting families with each step in the Early Intervention process. Arranging the evaluations.

· Facilitating and participating in the development of each service plan meeting. Securing service providers.

· Complying with the new telehealth regulations and guidelines provided through the department of mental health and hygiene. Providing families with resources outside of the early intervention program.

· Developing a transition plan with along side the families in preparation for the age out of Early Intervention.

· Coordinating and monitoring the delivery of services.

· Submitting service coordination billing, as well as documentation into the provide soft program.

• Enhancing family engagement in the NYC Early Intervention Program by providing families with in depth information about the Early Intervention program throughout each step of the process.
• Improved child outcomes by closely monitoring progress and adjusting intervention strategies along parent and clinician as needed.
• Streamlined communication between families, therapists, and other service providers to ensure a coordinated approach to care.
• Increased access to services for children in underserved areas through targeted outreach efforts.
• Facilitated service plan review, annual and amendment meeting within NYC Early Intervention guidelines.
• Assuring all evaluations and clinical sessions were arranged in a timely matter.

• Expediting the referral process for early intervention services, ensuring timely evaluations and interventions for at-risk children.
• Increased access to services for children in underserved areas through targeted outreach efforts.
• Advocating for the child's developmental needs.
• Helping families navigate the early intervention system.
