I will check vitals ( temperature, heart rate, blood pressure , O2), I will provide the best care to my patient, I will transfer my patient with best mechanic that will keep the patient and myself safe. ( Hoyer lift, sit-to-stand, slider board and gait belt) I will ambulate the patient with gait belt, walker or wheel chair. i will answer the the call light in appropriately time frame. If i see any changes on a patient I will immediately to the RN. I will help the patient with toileting, bed pan, bedside commode or change their brief, catheter care or colostomy care.

I will pass medicine to the patient. I will check Sugar before dinner and before bed. I will give insulin if the needed. I will help the CNA with their round when I finish my medicine pass. I will call the Ambulance if the patient needs to be seen at the hospital.
