  • Direct care of complex medical patients in their homes, managing their ventilator, supplies, education and coordination/communication back to their ordering physicians to ensure their needs are cared for
  • The patients range from severe end stage lung disease as well as neuromuscular and restrictive lung disease
  • Their ongoing care is coordinated by me with their various ordering physicians, as well as looking into other ways Avera HME can serve them in their home
  • Promoted by Upper Management to care for patients of the Milbank clinic and hospital once a week
  • Travel to patient’s homes in SD, IA and MN to serve the patient’s medical needs in their homes
  • Created a Respiratory Assessment form that prior to this has never existed within Avera HME
  • This is an assessment that allows the RT to go into the home at no charge to the patient, assess the patient and provide direct feedback to the physician on the patient and their home environment/equipment/medication management without the patient ever having to leave their home
  • These patients are not ventilator patients, but instead people who are struggling with various medical issues and need additional help in the home
  • Selected by our sales staff to accompany them to area clinics/hospitals and speak on behalf of our company about the clinical work we provide our patients
  • These meetings are composed of nurses, physicians and director of clinics/nursing
