Complete biopsychosocial assessments within five days of a patient's admission for hospice services. Provide emotional supportive counseling. Assist and advocate for patients and families as they navigate their healthcare experience while receiving end of life care. Make referrals for community services as needed. Participate in interdisciplinary team meetings on a bimonthly basis. Serve as field instructor and mentor for social work students.

Provide assistance with discharge planning from an acute care setting. Complete biopsychosocial assessments, identify patient and families strengths, needs, barriers and solutions for navigating through the healthcare system. Work with interdisciplinary teams to formulate effective treatment plans. Provide resources through the referral process, order equipment and arrange for a variety of outpatient services upon discharge from acute care.

Provide counseling services for clients experiencing mental illness. Formulate treatment plans with an interdisciplinary team to guide clients with effective therapeutic techniques and build coping skills. Facilitate group, family and individual counseling services.
