Performs a wide range of support services for the Case Management Staff. Assists with discharge planning, continuum placement, communication with insurance companies, and gathering of data. Works collaboratively with the Physician and other members of the health care team, supports patient care monitoring, coordination, and facilitation of patient care. Promotes quality outcomes, team accountability, and productivity, and becomes a link between the patient, the provider, the payer, and community resources. Demonstrates good communication skills, judgment, and maturity with patients, staff, and personnel. Interacts with patients in the neonate, infant, child, adolescent, adult, and geriatric age groups. Performs clinical duties in accordance with population-specific guidelines and adheres to National Patient Safety Goals as outlined in policy and procedures. Provides cross-coverage in all settings as required. Responds to all referrals on the same day received as evidenced by documentation in the medical record. Maintains detailed knowledge of community resources, PPO’s, and HMO’s to facilitate appropriate patient outcomes. Arrange appropriate discharge services for patients per physician orders including but not limited to Hospice, DME, Home Health Services, indigent medications from pharmacy, transportation home, follow-up appointments, etc. Completes transfer forms for patients moving within and outside the continuum of care, i.e., 4W, TCC, or other hospitals. Prepares DMA-6 from medical records for patients going to SNF. Involves synthesizing information from the medical record and completing the appropriate forms. Provides the requested information to nursing homes and third-party review agencies and provides follow-up for successful patient placement. Arrange DME and/or home health services for patients per physician orders. Arranges post-acute transportation in accordance with medical necessity; payor benefits; indigent process, e.g. taxi/Lyft.

Answer questions, following HIPAA guidelines while in the front area. Utilize the Patient Queue, Appointment Tool, and Patient Kiosk systems to effectively manage waiting room flow. Complete the registration process, placing the patient under the respective scheduler book for the provider on duty. Verify the patient or responsible party's identity using a government-issued source. Determine the method of payment, describing the options of payment available. Collect the patient or responsible party's insurance information, if applicable; collect payments and log in to the EHR. Scanning all appropriate documents. Escort the patient to the exam room and determine the patient's chief complaint. Obtain vital signs and patient history and enter the data in the EHR in designated clinics with completed competencies. Continue to process the patients in the waiting following appropriate clinic flow. Answer phones, respond to questions, and return patient/provider calls; complete daily patient callbacks. Assist the provider within specified company protocols to obtain specimens for testing as ordered by the provider on duty as well as report negative labs to patients per direction of the provider
