• Work with the providers and clinic staff to identify patients at high risk through transitions of care. This is to support the market initiative of reducing utilization's, including ER visits, hospital admissions, and hospital readmissions.
• Coordinate with providers to establish or update transition of care once released from ER or Hospital.
• Perform accurate and timely documentation in the electronic medical record.
• Participates in daily huddles and Patient Care Coordination (PCC) meetings for current patient updates to designated providers.
• Prepare accurate and timely reports (Hospital Transition, ER Grid, Readmits, etc.,) as required for weekly PCC meetings.
• Initiating Medication Reconciliation and Transition of Care documentation for tracking.
• Conducting motivational interviewing and Self-Management Goal setting and providing patient education.
• Assist with post discharge needs such as prescriptions, transportation, durable medical equipment (DME), appointments by creating and following up on social work referrals.
• Refer to case management for complex case needs, longitudinal needs, and/or disease management.

LVN-Allergy

• Collected patients vitals: temperature, blood pressure, pulse oximetry.
