I Work with members who have been approved for Home and Community-Based Services and who are on waivers through Medicaid. I have worked with members on the frail and elderly, physically disabled, intellectually and developmentally disabled, and brain injury waivers. For members who require personal care in their homes, a functional needs assessment is completed based on what the member's needs are, which determines how many hours the member is able to receive. Depending on what waiver the member is on, the waiver provides other services. My role is to provide the member with the names of the various providers for the services each waiver provides, and once they make their decision, I complete their service plan. I send their service plan to the chosen providers and member and then I have the authorizations tasked to be built for the services. In addition to the functional assessment and service plan, we are required to complete Personal Care Service Plan and four assessments at the initial and annual contacts with the member. We have quarterly contact with our members. Two face-to-face visits and two phone calls per year. Should our member have a change in care, health needs, or diagnosis, and require a change in hours, we will reassess the member and change the service plan based on their needs. For example, if a member goes into the hospital due to a fall and requires going to rehab after being discharged from the hospital, the member's hours will be reassessed before leaving rehab to ensure the member has increased support to ensure the member will be successful once they return home and does not return to the hospital. For members who are on the IDD waiver, we are one of the team members who provide support. We complete the same paperwork, assessments, and contact with our IDD members. I have been assigned a few waitlist members as well. These are members who are still on the waitlist for the IDD waiver, but we are offering them case management services. These members are eligible to receive the standard Medicaid services, along with the Value-Added Benefits through Sunflower, but they are not able to receive the full waiver benefits at this time.
I ensured that the foster parents' homes to which I was assigned were up to code, per KDHE, annually by completing walkthroughs of their homes and submitting a packet to KDHE. I completed home visits monthly with each child placed in my assigned foster home to ensure the safety and well-being of the children, and I documented each visit. I provided support to the foster parents as needed, encouraged positive communication between biological parents and foster parents in order to promote a healthy environment for children. I completed fingerprinting for background checks for new foster parents. When on call I handled calls from foster parents who were having struggles with the children they were caring for in their homes. I would talk with the children and foster parents to attempt to de-escalate the situation, keep everyone safe, and maintain the placement. There were times I needed to arrange transportation for a child who was moving to a new placement, or when a child's ride did not show up, so I had to fix the transportation issue.
