Patient Care

• Develop care plan, including discharge plans, in collaboration with health care team, including case managers, social workers, and physicians
• Initiate and manage preventive and rehabilitative nursing procedures, administer medications and IV therapies, and provide wound care, ostomy care, PICC line care, pain management, and gastrostomy tube care; strong clinical skills in preparing patients for invasive and non-invasive surgical procedures such as C-sections, episiotomies, and orthopedic surgery.

Patient Education and Staff Development

• Educate patients, families, and caregivers on diagnosis, treatment, and disease management.
• Orient and mentor new Field RNs on agency policies, home health patient care and requirements, and OASIS and Centers for Medicare and Medicaid Services (CMS).
• Past experience in precepting student nurses and new hires within hospital settings to develop and strengthen clinical competencies across a diverse range of specialties.

Nursing Administration

• Use Outcome and Assessment Information Set (OASIS) data collection process to ensure patient care aligns with Medicare requirements.
• Prepare and maintain accurate home health clinical notes and update case manager and primary physician on patient status and progress; enter data into electronic charts to drive continuity of care.
• Work with physicians to recertify patients for Medicare covered home health services by reviewing patient progress reports to ensure initial care plan.
