I started as one of the CCM coordinators and was promoted to manage and lead the whole CCM team.

• Monitored and managed patients with two or more chronic illnesses.
• Set appointments and worked with the clinic to monitor patients' labs, blood pressures, and completed health assessments to determine what referrals, if any, needed to be completed, such as home health referrals or physical therapy.
• Monitor and complete quality measures according to Medicare: A1c, fall risk, blood pressures, depression screenings, etc.

I was promoted to manager for the CCM/ACO team and these were my new job duties:

• Assign and update Google Sheets with patients who consent to CCM services.
• Assess patients on which health tier they fall under.
• Run reports against master list to determine what patients need MD visits to be attributed for ACO.
• Run reports to see which patients qualify, but are not enrolled in CCM services.
• Run reports for AWV that are due or upcoming.
• Work with the corporate office, the VP, the CEO, and the analytics teams to help grow our ACO population.
• Interview and train new hires.
• Manage all needs for a team of 15 CCM coordinators.

I started at this office with absolutely no dental experience at all, and ended up becoming a registered dental assistant, and moving up to the lead surgical assistant.

• I was responsible for assisting the doctor in all procedures, including teeth extractions, implants, fractures, biopsies, and any other oral or facial traumas.
• I was able to transition between a blended mixture of consultation appointments, scheduled surgeries, and even same-day add-on surgeries.
• I filled out referrals, MD releases, treatment plans, and sent prescriptions.

• I was responsible for overall patient care for the inmates.
• I took vitals and went over medications before the patient saw the provider.
• I performed EKGs for the inmates who had a cardiac history, or anyone who came in complaining of chest pain.
• Reasoning for such short employment was because I moved
• I ran the lab by doing routine lab tests and blood draws.
• I am crossed trained in the lab and can sub in for when our phlebotomist is out as well

I started at this job with no experience, and worked my way up to Lead MA. In this position, I worked scribing part-time for the doctor. I completed interviews and provided training for new medical assistants.

• My main responsibility was patient care in a fast-paced environment.
• Greeted and roomed patients and took vital signs.
• Completed blood draws and EKGs.
• My duties outside of contact patient care were to make pre-visit calls to inform our patients about what to expect at their appointments, instruct them on what to bring to their appointments, and collect medical records from different facilities to determine what further testing the patient may need to be cleared for surgeries.
• I cleaned and stocked rooms, and made sure our lab kits were fully stocked at the end of each day.
