  • Coordinated and created with the IT Director the New Visual Signature program and documents needed for eligibility.
  • Collaborated on the improvement of the Visual Signature forms and am a part of the Care Trax referral and care plan improvement group.

  • Collaborated on the development of a visual assessment tool with the goal of promoting health equity through improved client engagement and that invites shared decision-making from the initial visit. I also presented for the first time to the PMRP for feedback to the visual assessment tool.
  • A member of Quality improvement activity (PDSA) for eligibility appointment at the clinics which help to decrease the no-show/cancellations rate and increase the number of welcome visits completed monthly since April 2019.
  • Introduced the Proposal to Expedite Welcome Visits during COVID 19
