Managed two different insurance's and scheduled nurses for patient care. Maintained patient care by accurately maintaining daily, weekly and monthly visits. Assisted patients with PRN visits and monitored progress to meet goals. Collaborated with RN's, LVN's and Therapist to maintain adequate patient care for home health patients.

Assisted patients with their move in process into the community. I assisted in gathering necessary documentation for appropriate and safe move in to the community. I worked with patients care team to make sure our community was a safe fit for the patient to reside in.

Coordinated and implemented weekly and monthly activities and appointments for residents living in the community. Coordinated shopping trips, lunch outings and doctors appointments for our residents outside of the community. Maintained monthly activities for residents to enjoy within the community; for example bingo, movie nights, dinning services and informative safety meeting for residents. Maintained patients safety and promoted independence for residents within our community.

Assisted residents with their activities of daily living while promoting safety and independence. As a caregiver I assisted residents with daily grooming, toileting, showering and transporting throughout the community. Working as a Med-Tech I administered medications and implemented some tx under a Licensed Nurse. I worked with residents family, doctors and pharmacies to administer the correct medications as ordered. I also assisted in emergency situations with residents and EMS. I monitored and maintained residents care while promoting their independence and keeping them safe.

Assisted Enrolled students with completing necessary documentation for certain class requirements. Assisted disabled students with the enrollment process as well as transportation around campus. Assisted the RN on campus with BMI intake for health students and emergency situations on campus.

• Promoted independence in patients through walking exercises or assisting them with mobility devices like walkers or wheelchairs.
