Day Program Coordinator | [Colorado Department of Human Services] | [03/2023-06/2024]

In this role, I was a collaborative member of an interdisciplinary team responsible for the assessment, development, and implementation of Service Plans (SP) for individuals with developmental disabilities. I played an active role in the creation and implementation of formal and informal program plans, ensuring services were provided within established guidelines.

Key Responsibilities:

  • Led a team in the supervision and coordination of staff during day program hours, ensuring all services and supports were delivered according to individual Service Plans (ISSPs).
  • Managed the day program environment by organizing and maintaining program materials, schedules, and activities to promote the developmental growth of individuals.
  • Provided individual therapeutic and habilitative services for an assigned caseload, monitoring progress and ensuring effective implementation of program objectives.
  • Actively participated in interdisciplinary team meetings, contributing to the development of individual assessments and the planning process for each individual’s goals.
  • Ensured the safety, well-being, and rights of all individuals, performing physical interventions and redirection techniques when necessary and administering medication as prescribed.
  • Managed logistics for participants' transportation to medical appointments, community activities, and day program sites, ensuring safety and efficiency.
  • Provided on-site direction and training for support staff, ensuring they were properly trained in the implementation of program objectives, therapeutic techniques, and safety protocols.
  • Organized and scheduled community integration activities, facilitating opportunities for individuals to engage with the community and enhance their vocational skills.
  • Maintained accurate documentation, including assessments, team notes, and written reports, to track individuals’ progress and support the team’s decision-making process.

Employee Training and Staff Supervision:

  • Directed and supervised a team of support staff, ensuring that their work duties were aligned with program schedules and individual needs.
  • Conducted training sessions, monitored staff performance, and identified areas for improvement, reporting to supervisors as needed to ensure compliance with established guidelines.
  • Acted as a liaison between direct care staff and supervisors, providing regular updates on individual progress, potential concerns, and compliance issues.

Other Contributions:

  • Assisted in special projects and participated in training sessions to enhance skillsets and maintain up-to-date knowledge of relevant policies and procedures.
  • Contributed to the Supportive Employment program by providing job training and support to individuals seeking employment opportunities.
