Here I specialized in working with an intensive caseload of 12 families. This position provides support to clients: needing emergency food services, poverty stricken, or in search of resource for shelter, clothing, education, or employment. As Pathway of Hope Case Manager, I provided daily to bi-weekly support to families as parent strive to meet the goals the have set while in the program.

• Work directly with the Executive Director and administrative team on collaborative events and programs, such as life skills training, drug awareness, tutoring, workshops, self-esteem, and wellness groups
• We serviced individuals of all ages with and without developmental, behavioral, or intellectual needs.

• I currently work with 65 children/families in school and home settings
• My duties range between providing resources to assist with essential needs and structure in homes experiencing behavioral and academic barriers or financial hardship
• CSS also assesses needs and implements a treatment plan to improve functionality and health of youth/families and provides counseling as needed.

• Duties included adhering to deadlines with documentation, supervising 20-25 direct care staff, and making sure that all the mental, physical, and emotional needs of the 132 IDD individuals on my unit were met
• I collaborated with a team of nurses, behaviorists, dietitians, and paraprofessionals to implement the daily routine of the residential program.

• I worked as an Assistant House Manager for a youth group home for eight males with severe behavioral and personal care needs
• Assisted with daily living and life skills, such as cooking, cleaning, academic, and social activities.

• Provide at-home services to private residents and assist individuals with disabilities with their daily living skills and emotional needs.
