ESSENTIAL QUALIFICATIONS: Highly experienced with providing case management to diverse communities with significant needs around housing, mental health services, food insecurity, and/or disability issues. Ability to engage clients using harm reduction and motivational interviewing practices. Willingness to work in a team-based, multi-collaborative environment. Deep knowledge and comfort with addressing complex issues facing those that identify as LGBTQ, aging, disabled, unstably housed or houseless, with unresolved immigration status, and those addressing substance use and/or mental health and/or cognitive health challenges. Cultural humility towards issues concerning gender, aging, chronic health conditions, disabilities, and co-morbidities.

In 2014, Brothaz Connexion was recognized by the State of California Office of AIDS Administration as a locally developed HIV prevention and education intervention. Brothaz Connexion has served as the longest-running group-level intervention for Black MSM in Alameda County. This intervention has also served as an effective retention program for those who are living with HIV; providing opportunities for supportive services that promote health seeking behaviors and an increase in health literacy among its participants.
