Evaluates referred patients to assess their appropriateness for admission to an inpatient rehab program

Facilitates admission working with the rehab attending physician and the nursing team

Facilitates the transfer of appropriate clinical information to appropriate individuals in order for the physician to review/approve the admission and for clinical staff to

prepare to care for the patient

Completes clinical documentation and other paperwork related to the patient admission accurately and within required timeframes

Works with discharge planning, case management staff and the system inpatient rehab program to facilitate a smooth transition for the patient and family into rehab

• Provide therapy interventions to homebound clients in their personal environment
• Schedule and communicate with clients daily/weekly
• Assist office staff with referral management tasks, Complete insurance verification, Communicate with Dr
• Office for appropriate documentation, Update and organize EMR, Constant communication with Therapists, Clinical Manager and Administrator re: referrals, View patient documents for services needed and appropriate for Home Health
• Assist at outpatient sites when caseload is low.

• Followed plan of care to achieve the goals set by OTR with homebound clients
• Problem solved challenges for homebound clients for increased independence
• Communicated with other healthcare professionals, whom were involved in client care.

• Assisted OTR with implementing and modifying treatment plans in Outpatient setting designed to help clients function independently in Independent Living Unit, Assisted Living Unit, Restorative Unit and Dementia Unit
• Worked closely with Restorative staff in carryover of achieving highest level of independence for each client
• Provided in-services to management, nursing and caregivers on use of adaptive equipment, feeding techniques, transfer techniques and dressing techniques
• Provided in-services to all new hires on use of proper body mechanics.
• Assessed ADL's for clients of 150 bed skilled nursing, sub-acute rehab facility
• Collaborated with other disciplines providing continuum of comprehensive care
• Attended interdisciplinary meetings with nurses, physicians, social workers, dietary and other therapeutic staff.
