LPN Position in Grafton, ND. Hours are primarily Monday through Friday with flexibility needed depending on the needs of the people supported and department.

Implements the Overall Service Plans, Personal Outcome Measures, Waiver Survey guidelines, Center for Medicare and Medicaid (CMS) guideline and any other organization regulations or standards

Provides oversight to the health and medical needs for the organization.

* Coordinates all medical appointments for assigned caseload which may include general health, dental, psychiatric, vision, OT/PT, etc.
* Maintains appropriate documentation in medical records.
* Ensures appropriate medications and MARs are current and up to date at each site.
* Ensures people supported participate in directing health needs for themselves.
* Works in concert with managers, administrators and QDDPs to ensure education is relevant to the organization. 
* Completes follow up on all incident management forms related to medical incidences.
* Ensures basic assurances, quality assurance and quality enhancements are carried out throughout the organization.
* Participates in interdisciplinary team meetings and is ready and competent to provide medical expertise for each person on their caseload.
* Facilitates communication with medical professionals, pharmacies and other relevant medical resources.
* Assists people to advocate on behalf of themselves which include making their doctor appointments, choosing their own doctors and being involved in their own self-medication administration.
* Ensures responsiveness with all incident management matters for each person on their caseload (i.e., bruise of unknown, med errors, etc.).
  
Provides staff development.

* Provides agency training to relevant medical topics to the organization.
* Attends and participates in continuing education programs to keep the organization at the forefront of new medical advancements.
* Develops, maintains and trains staff on med certification and annual education as assigned.
* Attends and participates in departmental staff meetings.
* Updates and maintains Medical Administration Manual, medication certification expectations, medication administration practices and other agency medical procedures.
* Participates in quarterly North Dakota nursing meetings as assigned. 
* Participates in assigned committees. 
* Ensures all nurses within the organization maintain a high standard of services for their caseload. 
  
Provides feedback to the Director of Health Services on identified patterns for the company.

* Completes and maintains agency data.
* Assists in the coordination of monthly statistics as requested.
* Assists in tracking medication errors, bruise and injury, and other applicable medical data. 
* Conducts health services audits, reviews and observations as needed by the agency. 
* Identifies, recognizes and acts upon patterns of services to celebrate and patterns of services that need improvement.
* Graduate of state-approved school of nursing and currently licensed to practice as a Licensed Practical Nurse in North Dakota.
* One year experience as an LPN in a nursing home, hospital or developmental disability provider setting.
* Current valid driver's license.
* Ability to update and maintain records according to standards.
* Ability to plan, organize and facilitate effective communication through clear speaking and writing.
* 18 years of age.
* Current valid driver's license, access to a licensed vehicle and proof of motor vehicle liability insurance is required.
* Able to plan, organize and facilitate effective communication including effectively communicating through clear speaking and writing.
