• Had accepted an on-call position, However, part way through orientation, unfortunately acquired Covid which impacted orientation. This coupled with other circumstances, lead to my decision not to continue with AC at this time.

• Responded promptly to crisis symptoms, support needs, pt passings, and providing interventions/support as needed.
• Obtain orders as prescribed by physicians.
• Triage and manage multiple pt/family needs while prioritizing tasks based on urgency or acuity levels of patients' conditions.
• Educate patients and families about medications, treatments, and end of life care.
• Utilized critical thinking skills when responding to complex scenarios.

Assess referred patients for symptoms/disease processes to determine eligibility for hospice.

Extensive assessment/eval of diagnostics, symptoms, and medical history, reviewing all with hospice medical director.

Work with hospice physician to establish initial plan of care in correlation with primary diagnosis for comfort management at start of care.

Determination of DME needs to support safe mobility and care.

Education on end of life provided to pts/families with addition of supportive disciplines identified to meet clinical, personal, psychosocial, and spiritual needs.

Assisting families in determining and preparing options for loved one’s final care needs.

Field nurse currently in the on-call role covering Friday through Monday morning in care of the hospice pt. Triaging for pts/families during this most sensitive period of their lives, providing interventions in collaboration with the attending physician, symptom management, addressing crisis needs, nurse visits for assessment and interventions, giving support to families at time of a loved ones passing, participated in coordination of care with physicians and other clinical staff to prepare for treatment, carry out interventions and enhance continuum of care.

• Field nurse case managing 12-18 pts, completing assessments, medication and symptom management, providing end of life support with education, emotional support, meeting criteria and guidelines per Medicare codes of conduct. Weekly attendance in IDG for team conference with medical director. Later moved into admission nurse role, providing education related to hospice program services, performing detailed assessments and medical history review in collaboration with medical director and hospice team to ensure hospice criteria are met, identifying symptom management needs, interdisciplinary team member needs, making referrals as appropriate, responsible for completing documentation, entering orders to meet compliance,

• Staff nurse for general inpatient hospice facility, providing bedside care in the patient with crisis symptom management needs. Management of pca delivered pain medications, securing orders to meet pt needs, providing end of life support to pts/families, ensuring final arrangements are honored at time of passing.
• Coordinated care with physicians and other clinical staff to prepare for treatment, carry out interventions and enhance continuum of care.

• Same duties as Kindred Hospice
