- LPN Field Case Manager visiting Medicaid members in their homes , Assisted Living Facilities and Skilled Nursing Facilities. Working with identified members to assess their care needs and implementing interventions and Services to meet those needs and coordinating the services with monitoring and evaluating the Case management Plan with the Members Personal needs and request. Communication with Medical Providers as needed for Member follow-up. Performing Comprehensive 701B Assessments on members with Communication and Documentation To CARES for 701b and to maintain Level of Care Dates. Educator role in assisting with Training in House and in the Field with newly Hired Case Managers and Providing assistance with HCBS Case Mangers.
