  • Education includes all areas of hospice and palliative care- communication, symptom management, ethical and cultural considerations, psychosocial and spiritual support, and how to discuss difficult decisions including but not limited to: code status, no resuscitation, no antibiotics, no feeding tube/artificial feeding or hydration, no intubation, do not transport and allow natural death
  • Perform daily rounds at the inpatient facility in conjunction with inpatient staff to develop plan of care for General Inpatient and Routine Home Care patients
  • This includes: development of plan of care, prescription of indicated medications and therapies, review of plan of care for efficacy and update plan as indicated; communication with patient and extended family/support systems to provide psychosocial, emotional and spiritual support; provision of education on patient physical condition to assist patient and family in decision making and support to inpatient staff as indicated
  • Perform Face to Face visits as defined by Medicare Guidelines
  • This includes scheduled visits and new evaluation of patients with prior hospice services
  • Perform follow up assessment of patients as requested by clinical team for patients with complex problems and/or questionable hospice eligibility
  • Participation in Interdisciplinary team meetings for review of patients
