- Perform daily rounds at the inpatient facility in conjunction with inpatient staff to develop plan of care for General Inpatient and Routine Home Care patients
- This includes: development of plan of care, prescription of indicated medications and therapies, review of plan of care for efficacy and update plan as indicated; communication with patient and extended family/support systems to provide psychosocial, emotional and spiritual support; provision of education on patient physical condition to assist patient and family in decision making and support to inpatient staff as indicated
- Perform Face to Face visits as defined by Medicare Guidelines
