Strategic Medical Social Worker

  • Complete initial, quarterly, and annual Comprehensive Assessments, KDQOL (Kidney Disease Quality of Life) surveys, HRSN (Health related social needs assessment, GAD-7, PAM/CKCC, PHQ-2, PHQ-9 and depression screener followup screeners on all patients according to CMS regulatory standard.
  • Participate in QAI monthly, for Home therapies and in clinic, to determine root cause analysis in problem areas of missed/no show treatments, eligible patients active transplant waitlisted, advance directives, depression screeners, and patient KDQOL scores. Address areas of concerns based upon findings and implement solutions.
  • Maintains a caseload of 120 + patients amongst Carolina Dialysis Orange, Carolina Dialysis Mebane, Compass at Hawfields SNF and currently providing coverage for Siler City.
  • Knowledge in Home Therapy modalities as well as in center hemodialysis. Participate with RN, RD to ensure patient meets adequacy. Collaborate with KCA for patients transitioning from in center to home.
  • Basic knowledge of Medicaid, Emergency medicaid and Medicare, American Kidney Fund. Collaboration with IC to ensure patient coverage and affordability. Collaboration with DSS when necessary.
  • Follow patient hospitalizations and collaborate with hospital case managers for discharge needs upon returning to the dialysis clinic, SNF or elsewhere. Document post-hospitalization updates upon discharge.
  • Participate and collaborate with IDT (Nephrologist, RD and RN) in patient Plan of Care to ensure individualized treatment.
  • Place transplant referrals for eligible patients at appropriate hospitals and follow up with patients, transplant coordinators, and nephrologists. Follow patient through the evaluation process until successfully transplanted.
  • Conduct transplant assessment every 90 days on every patient.
  • Connect patients with community resources, ensuring access to essential services such as housing, healthcare, prescriptions assistance, financial assistance, vocational rehabilitation and food.
  • Provide supportive counseling, end of life discussions and crisis intervention services to address urgent concerns related to mental health, substance abuse, or domestic violence issues. Place appropriate referrals and provide resources.
  • Practice cultural competence through ongoing professional development opportunities, providing culturally sensitive services to diverse patient populations.
