• Evaluate effectiveness of patient care and identities any issues that affect desired resident outcomes
• Assess, review, and maintain that daily resident care needs are met, and appropriate resident care is given

• Assist physician with procedures and treatments (PEG tube care, colostomy care)

• Conduct reassessments of current members in their homes and skilled nursing facilities using Uniform Assessment System (UAS)
• Eligibility determination by utilizing clinical expertise and critical thinking skills
• Communicating and care coordination with home care agencies and Care Management Team
• Completing Patient review instruments/ screens and all required nursing assessments for active members to determine adjustments/updates in Care Plan
• Coordinating health care services, member education and preventative interventions.
