Objectives
After retiring from the Fairfax County Health Department on 12/31/24, I find that I am not ready to relinquish my nursing license or give up my role as a RN providing care and support to others.
The position of PT Hospice Coordinator resonated with me personally and professionally as I have seen the difficulty family members, medical providers and clients have in understanding what hospice support might provide.
I feel my extensive experience with care coordination and assisting clients and family members in challenging situations/ settings, would be invaluable in this role.
